Mobile Health and Wellness Program

The Mobile Health and Wellness Program (formerly known as the Richmond Health and Wellness Program) spearheaded by the VCU School of Nursing delivers weekly wellness clinics in nine community-based sites across the Richmond region that focus on health assessment, monitoring and coaching.

Our vision is to become a transformational agent in co-creating resident-centered education services through program development and outcomes measurement that is replicable and recognized locally, regionally and nationally.

The Mobile Health and Wellness Program is an innovative, collaborative care coordination model that seeks to improve health outcomes of communities, enhance the lives of individuals, decrease health care costs and educate future practitioners. Initiated in 2012, the program brings services to participants at or near their place of residence and is centered on assessment, chronic disease monitoring, wellness coaching and providing care coordination to ensure individuals can access the services they need to remain independent in the community.

During clinic sessions, VCU students, precepted by faculty from the VCU schools of Nursing, Pharmacy, Medicine and Social Work, as well as the departments of Occupational Therapy, Physical Therapy, Kinesiology and Health Sciences and Psychology, meet with individuals to understand their health or social concerns. Together, the participant, student and faculty develop a collaborative plan of care customized to the health conditions, social needs and resources of each individual. The program has continued to expand to address advance care planning, food insecurity, issues related to falls and mobility, screening for behavioral health issues including depression, anxiety and cognition, high-risk medication usage, and risk reduction related to opioid use.
